Entrepreneurs can apply for a certificate confirming that they have suffered losses as a result of the flood that occurred in the Dzierżoniów Municipality in September 2024. The document is issued by the Mayor of Dzierżoniów at the entrepreneur's request and confirms that the company has experienced damage related to a natural disaster.
The certificate is necessary, among others: in the process of applying for loans intended for entrepreneurs affected by floods (including loans up to PLN 50,000 for removing damage resulting from floods from the Karkonosze Regional Development Agency S.A.)
How to obtain a certificate?
To obtain a certificate, entrepreneurs should submit to the Dzierżoniów City Hall:
- Entrepreneur's application for issuance pursuant to Art. 44 section 2 point 1 of the Act of September 16, 2011 on special solutions related to removing the effects of floods (Journal of Laws of 2024, item 654), certificates,
- Information about the entrepreneur regarding the subject of the damage (Appendix No. 1 to the entrepreneur's application).

The application contains basic data of the applicant, such as: name of the entrepreneur, address and Tax Identification Number, period and place of damage (permanent or additional place of business). Information from the entrepreneur regarding the subject of the damage is a mandatory attachment to the application. It contains a brief description of losses in property, plant and equipment and current assets, such as:
- buildings and structures,
- machines and devices,
- equipment,
- goods and warehouse supplies.
The entrepreneur may attach photographic documentation and an estimate of damage made by an appraiser (if he has it) to the application.
The application should be submitted in paper form during the Office's opening hours at the Customer Service Office (BOK) of the City Hall in Dzierżoniów, Rynek 1.
The certificate will be issued immediately, no later than within 7 days of submitting the application.
